諮商晤談辦法
★辦理預約(★ Appointment Methods)
- 現場預約:學生直接至諮商中心(U棟303)辦理諮商預約。
- In-person appointment: Students may go directly to the Counseling Center (Building U, Room 303) to make a counseling appointment.
- 線上預約:學生至行政資訊系統填寫線上預約,線上預約請至「諮商預約」辦理。
- Online appointment: Students may fill out an online appointment form through the Administrative Information System. For online appointments, please go to “Counseling Appointment.”
- 電話預約:學生致電至諮商中心辦理諮商預約,電話與各系所個管老師之分機請見「 人員及職掌」。
- Phone appointment: Students may call the Counseling Center to make a counseling appointment. For the phone number and extension numbers of the case managers in each department, please refer to “Staff and Responsibilities.”
- 他人轉介:由學校教職員填寫「學生輔導轉介單」,諮商中心收到轉介單後,將進行評估,進行後續安排。
- Referral by others: School faculty or staff may fill out the “Student Counseling Referral Form.” After the Counseling Center receives the referral form, an assessment will be conducted and follow-up arrangements will be made.
★諮商方式(★ Counseling Method)
- 採預約制,請先完成預約或轉介程序,個管老師將做後續連繫及安排。
- Appointments are required. Please complete the appointment or referral procedure first. The case manager will then contact you for follow-up and arrangements.
★諮商流程(★ Counseling Process)
- 初次晤談:完成預約諮商的學生,將由輔導老師進行初次晤談,初談後經評估,依初談內容與現行諮商資源,評估是否開案。
- Initial interview: Students who complete a counseling appointment will first have an initial interview with a counselor. After the interview and assessment, a decision will be made, based on the interview content and current counseling resources, on whether to open a case.
- 正式諮商晤談:安排諮商時間,與輔導老師進行正式諮商晤談。
- Formal counseling session: A counseling time will be arranged for the student to have a formal counseling session with a counselor.
★諮商規定(★ Counseling Regulations)
- 諮商時間:諮商時間以每週一次、每次五十分鐘為原則。
- Counseling time: In principle, counseling sessions are held once a week, with each session lasting 50 minutes.
- 諮商次數:以八次為限,八次時進行結案評估。
- Number of sessions: A maximum of eight sessions is allowed. A case closure evaluation will be conducted at the eighth session.
- 諮商出席:若因故不能前來諮商,須至少於晤談時間前二十四小時,以電話或親自到本中心請假。一學期內請假次數以三次為限,若請假次數超過三次則取消諮商晤談。若兩次無故未到者,則取消諮商資格。如仍欲諮商,須重新至本中心預約。
- Attendance: If a student is unable to attend a counseling session, they must request leave at least 24 hours before the scheduled session time, either by phone or in person at the Center. The number of leave requests is limited to three times per semester. If the number of leave requests exceeds three times, counseling sessions will be canceled. If a student is absent without notice twice, their counseling eligibility will be canceled. If the student still wishes to receive counseling, they must make a new appointment at the Center.
- 停止諮商:學生有權利終止諮商,唯須先和專業輔導人員討論,並進行最後一次結案晤談。
- Termination of counseling: Students have the right to terminate counseling, but they must first discuss it with professional counseling staff and complete one final termination session.
